PdfReport is not using the default <mail> configuration. To have it use <mail>, the user needs to explictly specify it:

<mail name="mail_config">
  ...
</mail>

<health:PdfReport>
  ...
  <mail>${mail_config}</mail>
</health:PdfReport>
